Background: Proteus mirabilis is a species of bacteria that can cause a variety of diseases however, it is most frequently linked to bacteremia and complex UTIs. Patients with immunodeficiency anatomical anomalies and ongoing urinary catheterization are affected.The study aimed to investigate the multidrug-resistant bacteria Proteus mirabilis and its ability and relationship to biofilm production. Methods: 242 samples were collected from different clinical cases and the bacteria were investigated using appropriate culture media including MacConkey and blood agar as well as some biochemical tests and then the diagnosis of the isolates was confirmed using VITEK Compact 2 device. Results: About53 isolates of Proteus mirabilis were isolated of which 47 isolates (88.7%) were from urinary tract infections, five isolates (9.4%) from otitis media and one isolate (1.9%) from vaginitis. Susceptibility testing was performed against ten antibiotics belonging to different groups and the results showed that the highest resistance rate appeared against cefotaxime at 56.6%, followed by amikacin at 41.5% and amoxicillin-clavulanic acid at 36%. The results showed that the resistance rate of the isolates to some antibiotics was low, as the resistance rate of the isolates to Azeteronam. Was 5.7% and to imipenem was 3.8%, The results showed that23 isolates (43.4%) were resistant to Antibiotics. The results showed that 84.9% of the isolates were able to produce biofilms, 15.5% of them were strong biofilm producers, 17.8% of them were intermediate biofilm producers, 66.7% of them were weak biofilm producers Conclusion: Twenty-three isolates showed multiple antibiotic resistance due to their different resistance mechanisms and their ability to produce biofilms to varying degrees.
